Going overseas, need help to take care of dog when sick

Hi all, I'm planning to go overseas starting July for few years on work purposes. I'll be back in Malaysia about a month every year. I have a 9 year old German shepherd who's not that healthy anymore. I have family who can bathe him and feed him but I'm concerned that they may not have the time to take him to the vet if he's ill (about once a month). He's still a fairly active, strong dog. But he never bites people and is good with people. Can anyone advise:
(1) on any doctors willing to make house calls
(2) or anyone I could hire to take him for vet visits.
I am based in Kota Damansara. Any help would be hugely appreciated.
